Initial Cost to Community: Cost App & Training.
Phase 1 would be to initiate an App for vulnerable person awareness. All law enforcement, public health, mental health and commission on aging would utilize this App.
The App would identify vulnerable people by address, name, aliases, and also provided frequented addresses and contacts.
Associate (family & friends) of the person would report contacts on the App and a well being score. They could also make referrals to community services just by utilizing the App and checking the appropriate referral.
The community would have one agency managing the App 24/7. When vulnerable people have no capable associates, the agency could contact the vulnerable person through the app, text messaging, phone or in person. Secondly, every time any community service has contact with the vulnerable person or their address, it would be recorded on the app.
The management of the App would work like this:
1. All community services would be trained on the App. 2. All medical and mental health providers in the community would given awareness training about the App. 3. Nonprofit charitable organizations would be given awareness training about the App. 4. Community services would begin registering vulnerable people on the App. 5. Each vulnerable person would be given a risk score and an accessibility score. The risk score provides the maximum time distance between contacts and quality of contacts to ensure the persons continued well being. The accessibility score tells the level of physical or mental handicap and accessibility to resources such as the App. The App would also contain a photo, language and emergency contacts. 6. Then capable persons connected with the vulnerable person would be registered to assist in making contact with the vulnerable person. 7. A continuous assessment and quality control would occurred for six months to a year.
After successful implementation and trial of the App for vulnerable persons, the App would slowly expand to:
1. Those at risk for mental health crisis; 2. Those with mental health and medical illness that are at risk; 3. Those who are at risk for alcohol and substance abuse without sufficient support structure; 4. Those at risk for criminal behavior.
